  • Room and Board with minimum 3 meals per day, and a healthy snack  
  • Accommodations for handicap individuals  
  • Temperature controlled environment  
  • Personal hygiene products  
  • Twin bed with semiprivate room: twin bed, nightstand, and a shared closet (maximum 2 patients per room)  
  • Weekly laundered linens and blankets  
  • Access to no-fee laundry appliances  
  • Community entertainment room with television streaming, games, and structured social activities

HOUSING ASSISTANCE

CorBow Inc., DBA Papo Hernandez Respite, Rest and Recovery Program employees shall assist patients in applying for a myriad of services that will provide a fluid transition back into the larger community. Assistance shall include applying for adequate housing opportunities and/or referral to shelters and services designed for homeless or under housed.

ADMISSION CRITERIA

Admission criteria and authorization of individuals to participate in program will be decided and administered by Kern Health Systems. 

  1. All patients must have prior authorization from their medical insurance. 
  2. All patients shall undergo an initial evaluation to ensure patients needs can be effectively and adequately met by Medical Respite: Rest and Recovery Program. Intake staff shall perform an on-site evaluation of all referred candidates prior to acceptance. The Papo Hernandez Respite Home shall reserve the right to accept a referred client only if reasonably certain the needs of the client can be met at the Medical Respite level of care. Should a client e denied, a detailed written explanation shall be provided as to why upon request of insurance provider.
  3. Patients are not permitted to have an unauthorized leave of the premises during program care. An unauthorized absence or leave of the premises will be considered a refusal of services and patient shall undergo a new evaluation and re-approval from insurance to reenter the program. 
  4. All patients shall have a negative COVID 19 test within 48hrs of admittance.

EXCLUSIONS

At this time, the Medical Respite Program is not equipped to accept:

  1.  Bed ridden patients and/or those requiring meals to be provided in bed.
  2. Terminally ill hospice patients.
  3. Persons that are not able to transfer or complete Activities of Daily Living independently.
  4. Persons having impaired bowel and bladder function with an inability to self-manage their own hygiene.
  5. Cognitively impaired persons.
  6. Persons with untreated or severe mental illness.
  7. Persons convicted of a sex crime, arson, or a violent felony.
  8. Patients with paralysis will be considered on a case by case basis.

SCOPE OF CARE

BASIC QUALITY STANDARDS OUTLINED BY THE NATIONAL HEALTH CARE FOR THE HOMELESS COUNCIL (2016)

In partnership with Kern Health Systems, we shall strive to adopt standards outlined by the National Health Care for the Homeless Council (2016)  

  • Manage timely and safe transitions to medical respite from acute care, specialty care, and/or community settings. 
  • Provide safe and quality accommodations and environmental services. 
  • Administer high quality coordination and provides wrap around social and behavioral support services. 
  • Facilitate safe and appropriate care transitions from the medical respite to the community. 
  • Care is driven by quality improvement and performance outcomes the enable care providers to provide the most efficient, patient-centered care possible.

Provide transitional care housing for individuals with a concomitant medical issue in need of a safe environment for healing. Provide a supervised and supportive drug and alcohol-free sober living environment for homeless or under housed men and women and agrees to accept new referrals 7 days per week. The Respite Center will operate 24/7, ensuring security, meals, and access to care coordination. Through this collaboration, medical care, mental health, independent living services and other social services will be provided. The ultimate goal is to see each client regain wellness and be offered the steps to permanent housing and independent living.
